Assess To Support Limited support young people to develop their independence skills in a peaceful and nurturing environment. We are committed to providing high quality, person centred services that promote the independence, health and wellbeing and inclusion of the people that we support where they have choices and are supported to make wise decisions.

Young parents who need support to further develop their independence and prepare for the birth of their child. However, we cannot provide accommodation for the child when born but can support with outreach support subject to capacity.

Each young person has a personalised support package which is agreed between the young person and the social worker, with the keyworker. Each support package considers the needs of the young person, their goals, and realistic steps to reach their goals.

We consider every referral in detail and are happy to consider other circumstances should it be appropriate for the services provided and other residents.
Assess To Support Limited has a personalised programme for all young people however the content will cover all aspects of independent living. Practical workshops are facilitated with individuals and in a group setting to encourage residents to engage in learning and developing their knowledge.
Assess To Support Limited takes a person-centred and restorative approach providing individual support plans to thrive for independence or increased independence. This will include a varying levels of support infrastructure with a view of promoting and developing independence, health and wellbeing and inclusion.
The accommodation offered is shared, and will vary on supervision/support agreed by the Local Authority’s social worker or PA. We can offer 1:1 support, intermittent support and on call support only. This will be discussed at the initial referral point, placement planning meeting and a review meeting will be held shortly after placement to ensure that the correct level of supervision is in place for the young person.
There is no timescale on a young person staying with us, and this can be agreed by the Local Authority and the registered manager upon arrival and be kept under review regularly.
Every young person has a key worker who will work with them to identify needs/targets and review them on a regular basis. The programme will also be developed by the key worker initially.
Support plans are reviewed monthly by the key worker, young person and they will then give feedback to the social worker for their views.
Accommodations are based in the Hoyland area of Barnsley. Both accommodations are communal homes where residents live semi-independently and learn all the necessary skills to move onto independent living.
The homes are staffed for a minimum of 4 hours per day, up to 24 hours a day, with the focus on supporting individuals to look after themselves. All people placed have a Keyworker who works alongside them and the funding authority to establish a needs-led support plan in line with their support needs. We can also provide an ‘enhanced supported living’ package which includes one-to-one support and can be discussed at the point of referral.
Staff accommodation is on site and have access to CCTV internally and externally.
Both houses are spacious, modern and bedrooms have lockable doors and en-suite bathrooms. The homes have a large garden which is easily accessible to residents to use at any time.
